living assitance program

The SVNA Living Assistance Program is designed for those who need help to continue living comfortably and safely at home. The program is particularly reassuring to adult children concerned about an aged parent or other family member living alone. SVNA staff, the client and the family work together to determine the support and services that the client needs to maintain independence.

Services can include:

  • Care plan assessment to determine eligibility for assistance and need for services

  • Liaison with families at a distance, keeping them apprised of their loved one's wellbeing and alerting them to changes

  • Crisis intervention

  • Advice on preventive care

  • Home assessment and modification suggestions to make the clien's movement about the home easier and safer

  • Referrals to specialists in financial, legal, and medical matters, including money management, conservatorship or guardianship.

  • Counseling, including family and individual therapy

  • Arranging for and monitoring assistance in the home, including personal care, housekeeping, meal preparation, shopping, transportation to appointments or other errands

  • Initiating and supporting discussions about advance medical directives

  • Assistance in moving to or from a home, retirement complex, nursing home or other living situation
