Our mission is to provide high quality, home health services to all residents of the Northwest Corner of Connecticut, regardless of their ability to pay. Salisbury Visiting Nurse Association (SVNA) was established in 1904. It is not only the oldest rural visiting nurse agency in the state, but one of the most respected. SVNA is a non-profit organization, governed by a volunteer Board of Directors. The agency is Medicare and Medicaid certified and accredited by the Joint Commission on Hospitals and Healthcare. SVNA has been recognized for superior performance in patient satisfaction by a national best practice research organization. With a staff of nearly 50, SVNA provides health services in the home, 24 hours a day, seven days a week, 365 days a year. SVNA is the only visiting nurse agency in the area that has the capability of offering 24-hour care. Our skilled teams of registered nurses, physical, occupational and speech therapists, and a medical social worker treat patients in the security of their homes. SVNA home health aides provide the personal care and practical household help that enable our patients to remain comfortably at home. Our home health aides are available for an hour, several hours, an entire day or night or longer, depending on the needs of each patient. Many insurance plans cover home health aide services and hours beyond insurance coverage can be arranged. All SVNA home health aides are state-certified and are supervised by our registered nurses in helping to carry out the patientıs personal physicianıs plan of care. SVNA Hospice provides terminally ill patients with compassionate in-home care to enable them to live their final days as comfortably and fully as possible. The SVNA hospice team offers palliative care that includes symptom or pain control to enable patients in the late stages of disease to remain comfortable, alert and in control. This can include skilled nursing care, home health aide services, medications, medical equipment, spiritual support and volunteers. Our medical social worker is available for counseling and guidance for the patient and bereavement support for the family. SVNA is Medicare-certified to provide hospice care, and other insurance plans provide this coverage. Homemaker, companion or nanny services are also available through SVNA. Call us if you need someone to help prepare meals, clean or shop. Our companions are the perfect choice if a loved one needs a pleasant, reliable person to keep him or her company.
