Our Care Team

“The mission of Salisbury Visiting Nurse Association is to provide comprehensive, compassionate health care and hospice care at home and in the community.”

SVNA is a team of health care professionals committed to providing exceptional care in the home setting. Our team’s commitment and passion to meet our patients’ needs is at the core of everything we do. We involve the family and the medical team in developing a comprehensive plan of care for our patients. Health care provided in the safety and comfort of the home becomes a reality with our caring staff.

Our caring team includes:

Skilled Nursing: Our nursing team is comprised of registered nurses who have extensive nursing experience and a real commitment to providing the best care possible to our patients. At SVNA a registered nurse is available 24 hours a day/365 days a year.

Our team of nurses has experience in:

  • Cardiac care
  • Maternal/child health nursing
  • Wound care
  • Hospice care
  • Palliative care
  • IV therapy
  • Chronic disease management

Rehabilitation Team: Our team of physical, occupational and speech therapists work to restore maximum function of our patients. Our physical therapists work to improve patients’ strength, balance and endurance following illness, injury or orthopedic surgery. Our occupational therapists assist by helping to improve fine motor skills that are important in completing daily activities such as dressing, cooking, or by teaching sequencing techniques to help patients correctly take medications.

Medical Social Work: Our medical social worker acts as an advocate to coordinate needed community services and to help patients and families through the emotional and financial issues that can accompany a health crisis.

Home Health Aide Team: Our team of home health aides is committed to serving our patients and families with respect, and dignity, and strives to provide the best service possible. Home health aides assist our patients with daily personal care activities such as feeding, bathing, dressing and exercising.

SVNA Home Assistance Team: Our private duty services are offered by our SVNA Home Assistance team. The SVNA Home Assistance team provides clients with private care aides, companion and homemaker services. Tasks performed can include: bathing, grooming, laundry, meal preparation, socialization, errands, transportation and much more. Our program is headed up by Meghan Plouffe, Director who oversee a team of personal care aides. All aides are employed by SVNA, so they are fully insured and bonded. All employees must pass a background screening, drug testing and a license verification process, in addition to a “Caregiver Quality Assurance” assessment—all to help give you peace of mind knowing your loved one is safe and well cared for.

Business Office Team: Working behind the scenes, our office staff keeps our business, information technology and payroll processes flowing smoothly for our patients and staff members.
